Patient population

Unique opportunity for exposure to forensic and long-term patients. Forensic patients usually are admitted by court order for evaluation of competency to stand trial or criminal responsibility (as an aid to sentencing); others admitted after being found incompetent to stand trial or not guilty by reason of insanity (NGRI).

Rounds vary with teams. Usually 9:00 - 10:30 a.m.

8:00 a.m. to 4:00-6:00 p.m. (in theory)

On Wednesdays, no ELMHC: go directly to didactics

Call

No overnight call at ELMHC (done by moonlighters). Doctor on Call (DOC) duties twice a week (8 a.m. - 6 p.m.) Some call at McLean.

DOC Duties

Pick up beeper from overnight moonlighter at 8 a.m. sharp. Remain on ELMHC grounds during day. Respond to emergency calls. Do admits only before 3 p.m. (very unusual); for patients who arrive later, write a very brief assessment to ensure no medical/psychiatric acuity, and the moonlighter will do the complete evaluation. Give beeper to moonlighter at 6 p.m.

Medical Coverage

Garner Haupert sees patients M/W/F.

Documentation

Currently 1 note per week. Beware the chart police.

Discharges

1. Write discharge order

2. Write discharge note in chart

3. Complete face sheet

4. Dictate discharge summary within 2 weeks,

using tape recorder in medical records (5^th floor).
